Senior Case Manager

Location: Mendota Heights, Minnesota, USA; onsite full-time position

Salary: $55,100–$56,615.25 annually, based on experience, skills, and qualifications

What you will do

• Coordinate services and supports for vulnerable individuals living in the community.
• Conduct ongoing assessment, planning, referrals, service coordination, monitoring, consultation, and advocacy.
• Manage a direct caseload and coordinate multiple service needs.
• Help individuals and guardians make informed choices and decisions about support services.
• Educate clients and peers on available resources and self-directed services.
• Collaborate with organizations, families, and community members while representing the interests of supported individuals.

Requirements

• Bachelor’s degree in a human service-related field.
• Two years of experience working with individuals with disabilities or older adults.
• Two years of full-time experience in CADI/DD waiver case management.
• Knowledge of the local community and self-directed services.
• Strong oral and written communication skills.
• Valid driver’s license, automobile insurance, and ability to meet the requirements of an unlimited driving position.
