Intensive Case Management Caseworker

Location: Silver Spring, Maryland, United States; a combination of office, remote, and field work within the service delivery area, with occasional evening or weekend work.

Pay rate: $29.10 per hour.

What you will do

• Determine client eligibility, conduct intake assessments, and explain program expectations, outcomes, and confidentiality.
• Develop individualized SMART self-sufficiency plans and review client progress every six months.
• Help clients navigate healthcare, benefits, transportation, and social service systems, including scheduling and attending appointments.
• Build community outreach networks and coordinate with service providers, refugee coordinators, and health coordinators.
• Respond to client crises, coordinate financial assistance, and provide extended orientation on local services.
• Maintain accurate case files and document services in the Efforts to Outcomes database; train and lead volunteers or interns when needed.

Requirements

• Bachelor’s degree in social work, public health, or a related field preferred.
• At least 1–2 years of case management experience with similar populations or client needs.
• Strong relationship-building, diplomacy, networking, problem-solving, prioritization, and time-management skills.
• Experience working and communicating effectively in a multicultural environment.
• Fluent English, spoken and written, is required.
• Proficiency with Microsoft Office, internet research, digital tools, data entry, and problem-solving; valid driver’s license and access to a personal insured vehicle required.

• Proficiency in languages spoken by the client group.
• Bachelor’s degree in social work, public health, or a related discipline.
