What you will do

• Conduct intake assessments and determine client eligibility for Intensive Case Management programming.
• Develop and evaluate individualized SMART self-sufficiency plans for enrolled clients.
• Assist clients in navigating healthcare and social service systems, including benefit applications.
• Maintain a strong network of local service providers through community outreach.
• Respond to client crises and provide orientation on local transportation and healthcare systems.
• Maintain detailed case documentation using the Efforts to Outcomes (ETO) database.

Requirements

• Must be based in or able to work in Silver Spring, MD, USA.
• Minimum 1-2 years of case management experience with vulnerable populations.
• Bachelor’s degree in social work, public health, or a related field (preferred).
• Fluent English (spoken and written) is required.
• Valid driver’s license and access to a personal, insured vehicle.
• Proficiency in Microsoft Office and digital tools for research and data entry.
