TL;DR
Field Advanced Practice Provider - NP - Grand Rapids - PRN (Palliative Care): Providing in-home and virtual remote palliative care for adult patients with chronic and complex conditions in the Grand Rapids area with an accent on individualized care planning, clinical assessment, and family support. Focus on coordinating interdisciplinary care, managing treatment plans, educating patients and families, and maintaining compliant medical documentation.

Location: Michigan, United States of America; within a reasonable commuting distance of the posting location. Field-based work with travel to patient homes and occasional office attendance; in-home and virtual remote palliative care is provided in the Grand Rapids area.

What you will do

  • Provide clinical and non-clinical palliative care services in patient homes and through virtual visits.
  • Develop and implement individualized care and treatment plans for adults with chronic and complex conditions.
  • Assess patient history, physical condition, and diagnostics while aligning treatment with goals of care.
  • Address patients' and families' physical and psychosocial needs in collaboration with an interdisciplinary team.
  • Educate patients and families about medications, illness progression, nutrition, adherence, and crisis prevention.
  • Coordinate care with physicians and other providers, maintain medical records, and prescribe medication within state authority.

Requirements

  • Master of Science in Nursing.
  • Current unrestricted Nurse Practitioner or Physician Assistant license in the applicable state.
  • DEA registration or eligibility.
  • CPR certification.
  • Michigan licensure is required for the successful hire.
  • Travel may be required, and candidates must be within reasonable commuting distance of the posting location unless an accommodation is granted.

Nice to have

  • Two years of experience in home-based services, complex care, hospice, or palliative care.
  • Bedside hospital, oncology, emergency room, or intensive care unit experience.
  • Experience with Electronic Medical Records.
  • Bilingual English/Spanish skills.
  • Successful completion of a tuberculosis test for the Carelon Health business unit.
