Nurse Patient Advocate - Clinical Eligibility

Location: United States - Remote; 25–30 hours per week, between 10am and 4pm ET

Compensation: $28–$32 per hour

What you will do

• Assess patients’ health status, needs, and care preferences.
• Develop and coordinate individualized care plans with multidisciplinary healthcare teams.
• Educate patients and families about disease processes, treatment options, and self-care strategies.
• Advocate for timely access to healthcare services and treatments.
• Schedule appointments, manage referrals, and coordinate care across multiple providers.

Requirements

• Must be based in the United States and able to work remotely between 10am and 4pm ET.
• Active, unrestricted state license as an LPN, LVN, or RN.
• At least 2 years of clinical nursing experience.
• Knowledge of healthcare systems, care coordination, and patient advocacy principles.
• Strong communication, critical thinking, problem-solving, and patient education skills.
• Ability to work independently while collaborating with a multidisciplinary care team.

Nice to have

• Experience in case management, patient advocacy, home health, or chronic disease management.
• Experience working with Medicare patients and high-risk populations.
• Familiarity with electronic health records and telehealth platforms.
