Nurse Practitioner (1099 / PRN) (In-Home Assessments)

Location: Chicago, Illinois, United States; on-site field work with daily travel within a 55-mile radius of the assigned county

Pay: Approximately $100 per completed assessment plus paid mileage.

What you will do

• Conduct in-home Annual Wellness Visits and chronic condition assessments for Medicare members and other at-risk populations.
• Perform focused patient histories, physical examinations, vital-sign checks, medication reviews, and health screenings.
• Complete screenings including Hemoglobin A1C, spirometry, KED, DRE, and FIT.
• Educate members about preventive care and chronic disease management.
• Document visits in the EMR to support value-based care and risk adjustment.
• Work independently in the field while honoring confirmed visits and maintaining reliable monthly availability.

Requirements

• Active, unencumbered Nurse Practitioner license in the applicable state.
• Approximately 3 or more years of patient care experience preferred.
• Comfort with EMR systems and independent clinical work.
• Availability for at least approximately 30 hours per month on an ongoing basis.
• Willingness to obtain additional licensure if needed, with support provided.
• Ability to travel daily within a 55-mile radius of the assigned county.
