Nurse Practitioner (1099 / PRN) – In-Home Assessments

Location: Atlanta, Georgia, United States; on-site role involving daily travel within a 55-mile radius of the assigned county.

Pay: Approximately $100 per completed assessment, plus paid mileage.

What you will do

• Conduct in-home Annual Wellness Visits and chronic-condition assessments.
• Perform patient histories, physical examinations, vital-sign checks, medication reviews, and screenings.
• Complete screenings including Hemoglobin A1C, spirometry, KED, DRE, and FIT.
• Educate members about preventive care and chronic disease management.
• Document visits in the EMR to support value-based care and risk adjustment.
• Work independently while honoring confirmed visits and maintaining reliable availability.

Requirements

• Active, valid, and unencumbered Nurse Practitioner license in the state of practice.
• Reliable transportation for daily travel to members’ homes within the designated 55-mile area.
• Three or more years of patient-care experience is preferred, including primary care, adult, geriatric, or EMR experience.
• Comfort using EMR systems and working independently.
• Ongoing availability of approximately 30 hours per month, with preference for weekday and standard-hour schedules.
• Additional NP licensure may be requested and is supported by hirify.global.

Nice to have

• Previous in-home or risk-adjustment assessment experience.
• Previous 1099 or PRN experience.
• Bilingual communication skills.
