Nurse Care Coordinator (RN)

Location: Remote within the United States; active California RN licensure required. Work Monday-Friday from 12 PM to 9 PM PST with a one-hour break; occasional Saturday coverage may be scheduled in advance.

Salary: $70,000-$90,000 per year, depending on geographic location, with medical and ancillary benefits.

What you will do

• Coordinate, schedule, and reschedule clinical appointments, referrals, care transitions, and specialist services.
• Follow up with families to reduce care gaps and no-show rates while supporting continuity across the interdisciplinary care team.
• Provide evidence-based education about chronic conditions, medications, treatment plans, and care goals through messaging and phone.
• Manage medication prior authorizations, pharmacy communication, refill coordination, authorization tracking, and escalation of delays.
• Document clinical interactions across multiple EHR and care management platforms and report patient progress and care gaps.
• Participate in case conferences, team huddles, quality improvement meetings, and workflow improvement initiatives.

Requirements

• Active California Registered Nurse (RN) license in good standing required; an Associate's, Bachelor's, or Master's degree in Nursing is accepted.
• Proficiency with multiple technology platforms and electronic health record systems.
• Strong organizational, communication, and interpersonal skills.
• Ability to manage competing priorities and collaborate effectively in a remote, virtual environment.
• Comfort working with diverse, medically and socially complex pediatric populations.
• Reliable internet access and a dedicated workspace suitable for remote work required.

Nice to have

• Bilingual English/Spanish communication skills.
• Experience in pediatric, chronic care, or care coordination settings.
• Familiarity with prior authorization processes and payer requirements.
• Experience working in underserved or urban environments.
