What you will do

• Provide daily leadership, mentorship, and clinical guidance to the Colorado Care Management team.
• Serve as the primary clinical subject matter expert for Colorado operations.
• Review and approve care plans to ensure regulatory compliance and accuracy.
• Conduct routine chart audits and implement corrective actions to maintain audit readiness.
• Supervise, coach, and develop care managers through performance management and training.
• Partner with operations leadership to improve clinical workflows and standard operating procedures.

Requirements

• Active Registered Nurse (RN) license in good standing required.
• Bachelor of Science in Nursing (BSN) required.
• Minimum 3–5 years of clinical nursing experience in home- and community-based settings or case management.
• 2+ years in a clinical leadership or supervisory role, preferably within Medicaid-funded programs.
• 1+ year of experience developing or implementing clinical SOPs and process documentation.
• Must be based in or able to work in Denver, CO.

Nice to have

• Master’s degree in nursing, Healthcare Administration, or Public Health.
• Compact nursing licensure.
• Experience using Salesforce CRM.
