RN Care Manager – Fayetteville, NC (Hybrid - Primary remote)

Location: Fayetteville Hub, North Carolina, United States; hybrid with approximately 80% telephonic work and occasional travel for field or clinic visits, averaging once per week

Salary: $80,000–$93,000 annually

What you will do

• Manage a panel of clinically complex members across clinical programs, quality-gap initiatives, and utilization support.
• Complete clinical, self-efficacy, condition-specific, and behavioral health assessments, including PHQ-9, GAD-7, AUDIT, and DAST-10 screening.
• Develop and regularly reassess individualized care plans, addressing social needs and supporting members toward program graduation.
• Coordinate care with the integrated care team, external providers, community resources, and behavioral health programs.
• Conduct clinical visits and follow-ups, support chronic disease and psychiatric care, and provide medication reconciliation, administration, compliance, and education.
• Close prioritized quality gaps and maintain accurate documentation and ICD/CPT coding in electronic health records and care coordination platforms.

Requirements

• Graduate of an accredited school of nursing and hold an R.N. qualification.
• At least 3 years of professional experience.
• Strong critical thinking, independent clinical judgment, and ability to identify barriers and develop practical solutions.
• Strong written and verbal communication across phone, text, virtual, and in-person settings.
• Ability to use technology for remote member engagement and apply Motivational Interviewing and Trauma-Informed Care principles.
• Ability to work from the Fayetteville, North Carolina hub and travel for occasional field or clinic visits.
