Care Coach (Medicaid Care Management)

Location: Remote in Illinois or a border state within a commutable distance of Illinois; requires up to 25% field-based travel and occasional onsite meetings at hirify.global locations in Illinois. Core hours are primarily Monday-Friday, 8:00 AM-5:00 PM CST, within operating hours of 8:00 AM-7:00 PM CST.

Salary: $53,700-$72,600 per year

What you will do

• Conduct member outreach and risk-stratification screenings while meeting outreach and contact requirements.
• Identify needs and triggers for care management, chronic-condition programs, specialty services, and medical referrals.
• Coordinate healthcare services, resolve barriers to effective care, and connect members with community resources.
• Monitor member progress through assessments, data, conversations, and active care planning.
• Collaborate with providers, internal and external healthcare professionals, and multidisciplinary teams.
• Document member calls and care activities accurately while meeting regulatory, accreditation, confidentiality, and daily performance requirements.

Requirements

• One of the following: bachelor's degree in social sciences, social work, human services, or a related field; unrestricted Illinois LPN license; LPC license; or LSW license.
• Residence in Illinois or a border state within commuting distance of Illinois is required.
• At least 1 year of experience in healthcare and/or case management.
• At least 1 year of experience with Medicare and Medicaid recipients and/or managed care organizations.
• Valid state driver's license and personal vehicle liability insurance are required.
• Patient-facing work requires participation in hirify.global's tuberculosis screening program.

Nice to have

• Bilingual English and Spanish, Polish, Arabic, or Creole.
• Certified Case Manager certification or a master's degree in a health services-related field.
• Experience with electronic case notes, multiple documentation systems, health promotion, coaching, and wellness.
• Knowledge of community health and social service agencies, discharge planning, adult acute-care education, and basic medical terminology.
