Care Coordinator III (Medicaid)

Location: Remote within specified Illinois counties, with 10% travel to a local office for quarterly team meetings. Work schedule: Monday–Friday, 8:00 a.m.–5:00 p.m. Central Time.

Pay range: $20.39–$34.71 per hour.

What you will do

• Conduct phone outreach and home visits to engage members, discuss care plans, answer questions, and provide education.
• Coordinate care activities with members, caregivers, healthcare providers, community organizations, and care managers.
• Support member and provider inquiries, referrals, service needs, and continuity-of-care activities.
• Perform service assessments and screenings for members with complex needs, then document their care requirements.
• Maintain member records in accordance with state and regulatory requirements.
• Triage, adjust, and escalate complex requests while supporting care management training and development.

Requirements

• High school diploma or GED.
• 2–4 years of related experience.
• For the Illinois YouthCare plan: bachelor’s degree in nursing, social sciences, social work, or a related field.
• For the Illinois YouthCare plan: one year of supervised clinical experience in a human-services field.
• Must reside in one of the specified Illinois counties.
• Ability to work Monday–Friday during Central Time business hours and attend quarterly local-office meetings.
