Senior Care Manager (RN)

Location: Remote-NH; NH RN licensure is required

Salary: $75,300–$135,400 per year

What you will do

• Assess member needs and develop, monitor, and update personalized care plans and service plans.
• Coordinate care among members, families, caregivers, healthcare providers, specialists, and community resources.
• Conduct telephonic, digital, home, and other site outreach to assess needs and facilitate services.
• Educate members and caregivers about disease processes, care options, referrals, benefits, provider instructions, and care gaps.
• Review member data, referrals, and intake assessments to improve quality, operating performance, and regulatory compliance.
• Support clinical new hires through precepting, onboarding, competency checkpoints, coaching, and shadowing.

Requirements

• Registered Nurse licensure in New Hampshire or compact-state licensure is required.
• Degree from an accredited nursing school or a Bachelor's degree in Nursing.
• 4–6 years of related experience.
• Experience in both physical health and behavioral health is highly preferred.
• Ability to document care management activities and comply with state, federal, and third-party payer regulations.
