Senior Care Coach

Location: Remote Illinois; must reside in Illinois or a border state within a commutable distance of Illinois. The role requires up to 25% field travel and occasional onsite meetings at hirify.global locations in Illinois. Core hours are primarily Monday–Friday, 8:00 AM–5:00 PM CST.

Salary: $65,000–$88,600 per year, plus eligibility for a bonus incentive plan.

What you will do

• Provide coverage and support for Care Coordinators and document activities in the clinical system.
• Mentor team members, support training initiatives, and assist with new-hire onboarding and precepting.
• Conduct case audits, review compliance dashboards, prepare daily case reports, and support internal and external audits.
• Coordinate, monitor, and evaluate case management plans against members’ personal goals.
• Monitor member progress through assessments, data, conversations, and active care planning.
• Connect members and families with providers, community resources, payer sources, and interdisciplinary care teams.

Requirements

• One of the following: a bachelor’s degree in social sciences, social work, human services, or a related field; an unrestricted Illinois LPN license with one year of relevant assessment and service experience; an LPC license; or an LSW license.
• Residence in Illinois or a border state within a commutable distance of Illinois is required.
• At least 3 years of experience in healthcare and/or case management.
• At least 2 years of experience with Medicare or Medicaid recipients, long-term care, home and community-based services, or managed care organizations.
• Ability to use electronic medical records and other information systems.
• Valid state driver’s license and personal vehicle liability insurance are required.

Nice to have

• Master’s degree in social sciences, social work, human services, or a related field.
• CCM certification.
• Experience with health promotion, coaching, and wellness.
• Knowledge of community health, social service agencies, and additional community resources.
