Transitions of Care RN Case Manager

Location: Remote USA. Working hours are Monday–Friday, 8:30–5:00 MST/PST; candidates should be located in the CT, MT, or PT time zones.

Salary: $85,000–$100,000 per year.

What you will do

• Provide short-term, telephonic interdisciplinary care management for high-risk patients during the 30 days after discharge.
• Conduct assessments covering comorbidities, hospitalization history, medications, psychosocial factors, and patient preferences.
• Develop and evaluate individualized care plans with patients and caregivers.
• Educate patients about chronic conditions, warning signs, exacerbation plans, and medication adherence.
• Coordinate handoffs and follow-up with Nurse Practitioners, Physicians, primary care partners, and Devoted Medical.
• Coordinate community resources, post-treatment care, and durable medical equipment needs.

Requirements

• An unrestricted Compact RN license obtained in the United States, with the ability to secure additional state licenses.
• 3–4 years of RN care management or telephonic case management experience.
• Availability for Monday–Friday, 8:30–5:00 MST/PST working hours.
• Location in the CT, MT, or PT time zones within the USA.

Nice to have

• Experience building trust with patients and caregivers by phone, educating them, and coordinating care.
• Comfort working with performance and productivity metrics focused on quality outcomes and patient experience.
• Clinical analytical thinking and telephonic care or case management experience.
