Supervisor, Care Management - LTSS (Healthcare)

Location: Remote within Iowa, with an assigned southwest and south-central Iowa territory. Regular travel throughout the service area is required for in-person member visits, assessments, and care coordination.

Salary: $75,300–$135,400 per year

What you will do

• Supervise the long-term care management team and oversee day-to-day escalations involving members and providers.
• Maintain a small member caseload and conduct in-person visits, assessments, and care coordination activities.
• Monitor documentation for compliance with federal and state regulations and contractual requirements.
• Assign caseloads and work based on state requirements, staff experience, and member needs.
• Handle complex care cases, provide guidance and coaching, and improve member and provider experience.
• Support onboarding, hiring, training, performance evaluation, and change initiatives.

Requirements

• Bachelor's degree and 4+ years of related experience.
• For the Iowa plan: bachelor's degree and 4+ years of experience with the populations served, or an RN license with 6+ years of relevant experience.
• Experience with long-term care management, care coordination, or related member and provider services.
• Ability to work within the assigned Iowa territory and travel regularly for in-person member activities.
