RN, Senior LTSS Service Care Manager

Location: Hybrid remote work with required home and site visits in Brownsville, Harlingen, and San Benito, Texas. Schedule: Monday–Friday, 8:00 AM–5:00 PM CST. Travel for member assessments is required.

Salary: $36.21–$65.09 per hour

What you will do

• Assess complex and high-acuity members and develop personalized long-term care and service plans.
• Coordinate physicians, specialists, facilities, agencies, caregivers, community resources, and member services.
• Monitor member status, care-plan progress, complications, care gaps, and changes requiring updated services or a higher level of care.
• Educate members and families on disease processes, benefits, care options, referrals, and available community resources.
• Document care-management activities and member information in accordance with state, federal, and clinical requirements.
• Support quality improvement and may precept and coach new clinical hires.

Requirements

• Graduate of an accredited school of nursing.
• 4–6 years of related experience, preferably in RN care coordination or case management with adults aged 21 or older.
• Current RN state licensure and/or compact-state licensure, or current state nurse-practitioner licensure.
• Travel to conduct member assessments and periodic home or site visits is required.
• For Superior, Resource Utilization Group certification must be obtained within 90 days of hire.

Nice to have

• Bachelor of Nursing degree.
• Experience in home health, hospice, long-term care, or managed care.
