Case Manager, RN

Location: Telecommuter in the United States; Premera is hiring in listed U.S. states. Training and ongoing schedules follow Pacific Time.

Salary: $81,100–$137,900 national range; $92,400–$157,100 national plus range for higher-cost labor markets.

What you will do

• Engage members experiencing medical utilization challenges and build supportive relationships around sensitive health topics.
• Conduct holistic health assessments covering medical needs, social determinants of health, cultural influences, and barriers to lifestyle change.
• Develop member-centered case management plans with members, caregivers, and providers.
• Monitor progress, evaluate plan effectiveness, and update goals as members’ needs change.
• Coordinate communication across healthcare providers and deliver interventions using motivational interviewing techniques.
• Maintain accurate documentation that meets accreditation, contractual, and legal requirements.

Requirements

• Active Washington State or Compact RN or ARNP license permitting independent assessment.
• At least three years of post-licensure direct care experience.
• At least one year of post-licensure case management experience, including care coordination, care planning, discharge planning, or ongoing patient care management.
• Applicants residing outside Washington must obtain the appropriate Washington State license and apply within 10 days of acceptance.
• Internet access with a minimum download speed of 75 Mbps is required.

Nice to have

• Certified Case Manager certification, bachelor’s or master’s degree, and four or more years of direct care experience across multiple populations or settings.
• Motivational interviewing experience and chronic condition certifications.
• Integrated Case Management, Chemical Dependency Professional, or LEAN training.
