LTSS Service Care Manager - Behavioral Health

Location: Remote-TX, with service delivery in Spring, Texas and surrounding counties. Hybrid work includes work from home and in-person member engagement through frequent home or site visits, with up to 75% monthly travel. Work schedule: Monday–Friday, 8:00 am–5:00 pm CST; no on-call, evening, weekend, or holiday work.

Salary: $27.02–$48.55 per hour.

What you will do

• Evaluate complex, high-risk members with mental and behavioral health needs and develop or contribute to individualized care and service plans.
• Coordinate medical, behavioral, and social services, including housing, transportation, meals, employment, foster care, and activities of daily living.
• Act as a liaison and advocate for members and families while collaborating with physicians, facilities, agencies, providers, and community resources.
• Conduct home and site visits, assess member needs, coordinate referrals and authorizations, and ensure services are accessible.
• Educate members, families, and caregivers on preventive care, treatment options, medications, referrals, benefits, and provider instructions.
• Document care management activities, follow clinical and regulatory guidelines, support crisis and de-escalation activities, and identify quality improvement opportunities.

Requirements

• Master’s degree in Mental Health Counseling or Social Work.
• 2–4 years of related experience; experience with case management, high-volume caseloads, and field-based work is valuable.
• Unrestricted behavioral health professional license in the applicable state is required, such as LCSW, LMFT, LMHC, or LPC, according to state contract requirements.
• Strong clinical documentation, communication, problem-solving, organizational, and time-management skills.
• Proficiency with web-based applications and care management systems, including TruCare and Microsoft Office applications.
• For Superior Health Plan, a Department of Family & Protective Services background check is required.

Nice to have

• 4+ years of case management experience in child welfare, adoption, foster care, Child Protective Services, pediatric acute care, inpatient treatment, schools, or local or state mental health agencies.
• Experience supporting Foster Care Medicaid recipients and collaborating with medical and behavioral health providers.
• Experience in a managed care environment involving federal or state-sponsored government programs.
• Bilingual English/Spanish skills.
