LTSS Service Care Coordinator (Healthcare)

Location: Remote Iowa, United States; Monday–Friday, 8:00 AM–5:00 PM CST. Occasional on-site visits may be required.

Salary: $22.94–$38.79 per hour.

What you will do

• Assess member needs, available resources, and appropriate care options for long-term care.
• Develop and monitor personalized service plans and coordinate providers, specialists, and community resources.
• Coordinate communication between members, families or caregivers, healthcare providers, and community partners.
• Educate members and caregivers about services, referrals, provider instructions, and healthcare benefits.
• Document member information and care-management activities in compliance with state, federal, and payer requirements.
• Provide recommendations to improve quality, service delivery, and cost-effectiveness.

Requirements

• Must be based in Iowa for the remote Iowa plan.
• Bachelor’s degree and at least one year of related experience, or equivalent experience.
• Experience with populations served; direct experience with individuals with disabilities or vulnerable populations with chronic or complex conditions may be required for specific plans.
• Role-specific qualifications may include an Illinois RN license, a relevant human services degree, LPN experience, social work credentials, or case-management experience.
• Relevant experience may include working with addictive or dysfunctional family systems, racial and ethnic minorities, LGBTQ+ people, people with AIDS, and substance abusers.
