Care Navigator (Healthcare)

Location: Remote within the United States, with preference for candidates residing in California; availability to work Pacific Standard Time (PST) hours is required. The role may include on-site visits to assess member needs and collaborate with providers.

Salary: $22.94–$38.79 per hour

What you will do

• Evaluate member needs, barriers to care, available resources, and potential care outcomes.
• Develop or contribute to personalized care plans and identify appropriate providers, specialists, and community resources.
• Coordinate communication between members, caregivers, healthcare providers, and care management teams.
• Provide psychosocial, resource, and benefits support related to housing, food, employment, education, independent living, and other services.
• Monitor progress toward care-plan goals, support plan revisions, and refer members for additional care management evaluation when needed.
• Document member information and care management activities in compliance with state, federal, and payer requirements.

Requirements

• Bachelor’s degree and 2–4 years of related experience, or equivalent applicable experience.
• Graduation from an accredited school of nursing is required for candidates holding clinical licensure.
• Ability to work Pacific Standard Time (PST) hours.
• Ability to coordinate care, educate members and caregivers, and navigate healthcare and community resources.
