Lead Fullstack Engineer - Backend Focus (AI)

Location: Must live close to one of the designated U.S. IT locations: New York City, Louisville, Tampa, Fort Lauderdale, Dallas/Frisco, Boston, Washington, D.C., Atlanta, Nashville, Chicago, or Charlotte.

Salary: $129,300–$177,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Design and lead development of backend services, REST and GraphQL APIs, and service-to-service integrations for CenterWell’s AI product stack.
• Establish authentication, authorization, rate-limiting, PHI access-control, audit-logging, and HIPAA-compliant platform patterns.
• Implement reusable AI and LLM integration patterns, including routing, streaming responses, tool orchestration, and model-output handling.
• Build logging, monitoring, and alerting infrastructure for scalable and reliable platform operations.
• Deliver full-stack product features using TypeScript, Next.js, and React.
• Lead architecture reviews and define backend engineering standards.

Requirements

• Bachelor’s or master’s degree in Computer Science, Engineering, or a related field, or equivalent experience.
• 8+ years of software engineering experience with deep backend and platform engineering expertise.
• Strong proficiency in TypeScript and/or Python, with experience building scalable production REST or GraphQL APIs.
• Knowledge of OAuth2, OIDC, API security, authorization-model design, and secure handling of PHI under HIPAA.
• Experience with Next.js and React for full-stack feature development.
• Hands-on experience with AI-assisted development tools and LLM coding harnesses.

Nice to have

• Experience in healthcare, financial services, or another highly regulated industry.
• Exposure to event-driven architecture, microservices, or API gateway patterns at scale.
• Familiarity with AI/LLM integration, streaming APIs, and tool-use orchestration.
• Experience in a greenfield or team-founding engineering environment.
