RN Care Manager

Location: San Leandro, California, United States; work is primarily based in the Habitat center with visits to participants' homes and travel between work sites or community locations as needed. Estimated start date: November 30, 2026.

Salary: $58.00–$68.00 per hour, plus a target annual bonus of 5% of annual salary and possible additional incentive compensation.

What you will do

• Deliver personalized longitudinal nursing care to a panel of PACE participants in center and home settings.
• Conduct physical, psychosocial, and behavioral assessments and contribute clinical recommendations during Interdisciplinary Team meetings.
• Implement and document care plans, respond to physician orders, administer medications, and provide therapeutic interventions.
• Coordinate complex discharges, transfers, referrals, medications, medical equipment, supplies, and transitions of care.
• Triage outpatient needs, initiate therapies within scope of practice, and escalate care with medical providers when needed.
• Provide education to participants, caregivers, and colleagues; perform wound care, IV therapies, phlebotomy, and other RN-scope procedures.

Requirements

• Unencumbered California Registered Nurse license required.
• Graduation from an accredited nursing school or equivalent experience.
• 2–4 years of clinical RN experience with medically complex or older-adult populations, including at least one year working with frail or elderly people.
• 1–2 years of case management experience and strong clinical knowledge of chronic disease management and complex geriatric care.
• Experience with wound care, IVs, phlebotomy, colostomy or ileostomy care, and related clinical interventions.
• Current CPR/BLS certification required, or certification must be obtained within 30 days of employment; authorization to work in the United States is verified through E-Verify.

Nice to have

• BSN degree.
• PACE and/or ADCH/CBAS experience.
• State-issued driver's license, personal transportation, and auto insurance.
• Bilingual Spanish/English or Cantonese/English; other language abilities are also a plus.
