RN Care Manager (North Carolina)

Location: North Carolina, United States — remote, with travel within the local community for home-based member visits and visits to hospitals and rehabilitation centers

Salary: $71,000–$90,500 annual

What you will do

• Manage a panel of rising- and high-intensity members and communicate program expectations, goals, and duration.
• Complete clinical, self-efficacy, condition-specific, and behavioral health assessments, including PHQ-9, GAD-7, AUDIT, and DAST-10.
• Develop and monitor individualized care plans, conduct clinical visits and follow-ups, and coordinate providers and specialized resources.
• Participate in case conferences and collaborate with Community Health Partners, community health workers, and interdisciplinary care teams.
• Provide chronic disease, behavioral health, medication management, and health maintenance support.
• Address quality gaps and maintain complete clinical documentation and ICD/CPT coding in electronic health records and care coordination tools.

Requirements

• Active, unrestricted Registered Nurse license in the state where employment is sought with Cityblock.
• At least 3 years of clinical experience with adult and/or geriatric patients with co-occurring chronic medical and behavioral health conditions.
• Graduate of an accredited school of nursing.
• Experience with documentation, care planning, care coordination, interdisciplinary teamwork, and clinical writing.
• Prior experience or willingness to train in wound care, venipuncture and phlebotomy, medication administration, and care for diabetes, hypertension, CHF, and depression.
