Case Manager, RN (Healthcare)

Location: Telecommuter within the United States; Premera is hiring in specified U.S. states. The role requires an active Washington State or Compact RN or ARNP license. Candidates residing outside Washington must obtain the appropriate Washington license and apply within 10 days of acceptance. Training runs Monday-Friday, 8 am-5 pm Pacific Time for the first six weeks; the ongoing 40-hour schedule includes five hours of extended-hours coverage.

Salary: $80,200-$125,600 national annual range; $84,200-$143,100 national plus annual range for higher-cost markets including Western Washington and Alaska.

What you will do

• Engage members facing medical utilization disorders and build relationships around sensitive health topics.
• Conduct holistic health assessments, identifying medical needs, social determinants of health, and cultural factors.
• Develop member-centered case management plans with members, caregivers, and providers.
• Monitor and update care plans, goals, and progress toward improved health outcomes.
• Coordinate communication across the healthcare team and provide interventions using motivational interviewing.
• Document assessments, interventions, and care activities accurately and in accordance with accreditation, contractual, and legal requirements.

Requirements

• Active Washington State or Compact RN or ARNP license permitting independent assessments.
• At least three years of post-licensure direct care experience.
• At least one year of post-licensure case management experience in care coordination, care planning, discharge planning, or longitudinal patient care management.
• Residence in an eligible U.S. state and availability during Pacific Time coverage hours.
• Home internet service with a minimum download speed of 75 Mbps.

Nice to have

• Certified Case Manager certification; certification is required within 24 months if not held at hire.
• Bachelor’s or Master’s degree.
• Four or more years of direct care experience across multiple patient populations, conditions, or care settings.
• Motivational interviewing experience, chronic condition certifications, ICM or CDP certification, and LEAN training.
