Case Manager, RN

Location: Telecommuter within one of the listed U.S. states: Alaska, Arizona, Arkansas, California, Colorado, Florida, Georgia, Idaho, Iowa, Kansas, Kentucky, Maine, Michigan, Minnesota, Missouri, Montana, Nevada, New Hampshire, New Mexico, North Carolina, Oklahoma, Oregon, South Carolina, South Dakota, Tennessee, Texas, Utah, Washington, or Wisconsin. Training is Monday through Friday, 8 a.m. to 5 p.m. Pacific Time for the first six weeks. The ongoing 40-hour schedule includes five hours of extended-hours coverage between Monday and Friday, 5–7 p.m., or Saturday, 9 a.m.–2 p.m., Pacific Time.

Salary: $81,100–$137,900 national range; $92,400–$157,100 national plus range for higher-cost labor markets including Western Washington and Alaska.

What you will do

• Engage members experiencing medical utilization disorders and build supportive relationships around sensitive health topics.
• Conduct holistic health assessments, identify social determinants of health and cultural influences, and define member needs.
• Develop member-centered case management plans with members, caregivers, and providers, including goals and barrier-resolution strategies.
• Monitor and update care plans, evaluate progress, and coordinate communication across the healthcare team.
• Use motivational interviewing and other interventions to promote behavior change and strengthen natural support systems.
• Maintain accurate documentation that meets accreditation, contractual, and legal requirements.

Requirements

• Active Washington State or compact RN or ARNP license allowing independent assessments.
• Applicants residing outside Washington must obtain the appropriate Washington State license and apply within 10 days of acceptance.
• At least three years of post-licensure direct-care experience.
• At least one year of post-licensure case management experience involving care coordination, care planning, discharge planning, or longitudinal patient-care management.
• Internet access with a minimum download speed of 75 Mbps.

Nice to have

• Certified Case Manager certification; certification must be obtained within 24 months of hire if not held at hire.
• Bachelor’s or master’s degree and four or more years of direct-care experience across multiple patient populations, conditions, or care settings.
• Motivational interviewing experience and chronic condition certifications.
• Integrated Case Management, Chemical Dependency Professional, or LEAN training.
