Case Manager - Hudson

Location: Remote position supporting members in the Hudson/Baldwin/River Falls area, Wisconsin, with face-to-face member visits and up to 40% travel. Work hours are Monday–Friday, 8:00 a.m.–4:30 p.m. CST; occasional travel to hirify.global offices for training or meetings may be required.

Salary: $53,700–$72,600 per year.

What you will do

• Assess members’ health, safety, strengths, interests, and preferences to develop comprehensive Member Care Plans.
• Coordinate with Field Care Nurses and arrange services that support members in the least restrictive environment.
• Conduct enrollment and recurring social assessments, quarterly in-person visits, and monthly phone contacts.
• Arrange social integration, community resource, employment, housing, and other non-medical support services.
• Evaluate risk factors, educate members, ensure cost-effective service delivery, and maintain case notes, authorizations, and care plan updates.

Requirements

• Bachelor’s degree in human services or a related field plus at least one year of experience serving frail elders or adults with intellectual, developmental, or physical disabilities; alternatively, a bachelor’s degree in another field plus at least three years of relevant experience.
• Valid state driver’s license and personal vehicle liability insurance with minimum 100/300/100 limits required.
• Ability to conduct patient-facing work and complete required TB screening.
• Ability to work from a dedicated home workspace protecting member PHI and HIPAA information, with internet speeds of at least 25 Mbps download and 10 Mbps upload.
• Preferred: residence within 45 minutes of the assigned coverage area, case management experience, electronic case-note documentation experience, and knowledge of community health and social service agencies.
