RN Care Manager (Healthcare)

Location: Greensboro Hub, North Carolina; primarily remote with required on-site components based on member and business needs

Salary: $85,000–$95,000 per year

What you will do

• Manage a panel of clinically complex members across clinical programs, quality initiatives, and emergency department and inpatient utilization support.
• Complete clinical, condition-specific, self-efficacy, and behavioral health assessments, including PHQ-9, GAD-7, AUDIT, and DAST-10 screening.
• Develop and regularly reassess member care plans, address social needs, and support progress toward program and pathway graduation.
• Conduct clinical visits, follow-ups, examinations when appropriate, medication reconciliation, administration, compliance, and education.
• Lead or participate in case conferences and coordinate care with the integrated care team, external providers, community partners, and behavioral health programs.
• Close quality gaps and maintain accurate, timely chart documentation and ICD or CPT coding using electronic health records and care facilitation tools.

Requirements

• Graduate of an accredited school of nursing and licensed as a Registered Nurse.
• 3+ years of professional experience.
• Strong critical thinking, independent clinical judgment, problem-solving, and ability to identify barriers to care.
• Strong written and verbal communication across phone, text, virtual, and in-person settings, with comfort using technology to engage members remotely.
• Ability to apply Motivational Interviewing and Trauma-Informed Care principles and explain clinical information to non-clinical audiences.
• Completion of medical clearance requirements, including evidence of immunity to MMR, Hepatitis B, and Varicella and a TB screen, unless an approved medical or religious accommodation applies.
