RN, Field Care Manager - Central Region, Virginia

Location: Must reside in the Central Region of the Commonwealth of Virginia, including Richmond, Chesterfield, Colonial Heights, Petersburg, Prince George, Hopewell, Charles City, or nearby surrounding areas. Field position with 50–75% travel to members' homes, facilities, and providers; occasional onsite meetings in Glen Allen, VA. Workdays are Monday–Friday, 8:00am–5:00pm EST.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Conduct telephonic and face-to-face assessments for long-term services and support and aged, blind, and disabled members.
• Review medical complexity and develop individualized member care plans.
• Use evidence-based approaches to address members' physical, environmental, and psychosocial health needs.
• Connect members and families with appropriate healthcare, social service, and community resources.
• Identify and resolve barriers to effective care while monitoring progress toward desired outcomes.
• Make autonomous decisions and manage varied assignments with minimal direction.

Requirements

• Active Registered Nurse license in the Commonwealth of Virginia without disciplinary action.
• At least two years of healthcare and/or case management experience.
• At least one year of experience working with adults and children at risk for chronic medical conditions and high social needs.
• Knowledge of community health, social service agencies, and other community resources.
• Intermediate to advanced computer skills, including Microsoft Word, Outlook, Excel, electronic medical records, and web-based information systems.
• Valid state driver's license, personal vehicle liability insurance, ability to travel to facilities and homes, and completion of required TB screening.

Nice to have

• Bachelor of Science in Nursing and Certified Case Manager certification.
• Managed care, disease management, health promotion, coaching, or wellness experience.
• Three to five years of in-home assessment and care coordination experience.
• Motivational Interviewing certification or knowledge.
• Bilingual or multilingual ability, including English/Spanish, Arabic, Vietnamese, Amharic, Urdu, or another language.
