Case Manager, Behavioral Health

Location: Telecommuter within the United States; Premera is hiring in specified U.S. states. Individuals residing outside Washington must obtain the appropriate Washington State license. Training is Monday-Friday, 8 am-5 pm Pacific Time for the first six weeks; the ongoing 40-hour schedule includes five hours of extended-hours coverage in Pacific Time.

Salary: $80,200-$125,600 national range; $84,200-$143,100 national plus range for higher-cost markets including Western Washington and Alaska.

What you will do

• Engage members experiencing mental health and substance use disorders and build supportive relationships.
• Conduct holistic health assessments and identify social determinants of health and cultural factors affecting care.
• Develop member-centered case management plans with goals, barrier-reduction strategies, caregivers, and providers.
• Monitor and update care plans while tracking progress toward health and self-care goals.
• Coordinate communication across the healthcare team and provide interventions using motivational interviewing and behavior-change techniques.
• Maintain concise, accurate, and timely documentation that meets accreditation, contractual, and legal requirements.

Requirements

• Active Washington State or Compact license permitting independent assessments, including LMFT, LICSW, LMHC, LPC, or another CCMC-eligible healthcare license.
• Three years of post-licensure direct care experience.
• One year of post-licensure case management experience involving care coordination, care planning, discharge planning, or ongoing patient care management.
• Individuals residing outside Washington must apply for the appropriate Washington State license within 10 days of acceptance.
• Internet connection of at least 75 Mbps download speed is required.

Nice to have

• Certified Case Manager certification; certification must be obtained within 24 months of hire if not held at hire.
• Four or more years of direct care experience across multiple patient populations, conditions, or care settings.
• Motivational interviewing, chronic condition certifications, Integrated Case Management certification, Chemical Dependency Professional certification, or LEAN training.
