Salary: $94,424–$115,928 annually for the Hampton Roads, VA area.

What you will do

  • Collect and assess patient information, develop time-sensitive treatment plans, and monitor progress toward care goals.
  • Provide telephone-based nursing assessments using established protocols to coordinate appropriate and cost-effective care.
  • Oversee discharge planning and arrange referrals to suitable healthcare providers and facilities.
  • Use utilization review criteria, VA/DOD Clinical Practice Guidelines, research tools, and healthcare data to support evidence-based case management.
  • Negotiate and administer provider agreements while coordinating with healthcare professionals, vendors, and Military Health System stakeholders.
  • Prepare outcome analyses, presentations, briefings, and educational programs for peers and senior stakeholders.

Requirements

  • U.S. citizenship or permanent resident status is required.
  • A Secret or Top Secret clearance is required, or the ability to obtain a Secret clearance.
  • Current, full, active, and unrestricted Registered Nurse license in a U.S. state, the District of Columbia, Guam, Puerto Rico, or the U.S. Virgin Islands.
  • At least two years of nursing experience and case manager certification from a recognized organization, or the ability to obtain certification within the first year.
  • Fluent written and spoken English is required for clinical communication, presentations, research, and coordination with healthcare stakeholders.
  • Current Basic Cardiac Life Support certification from the American Heart Association, with proof of training and credentials.
