RN Case Manager (Healthcare)

Location: Hybrid in Detroit, Michigan

Salary: $71,000–$90,500 annual

What you will do

• Manage a panel of rising- and high-intensity members and communicate program expectations, goals, and duration.
• Complete clinical, condition-specific, self-efficacy, and behavioral health assessments, including PHQ-9, GAD-7, AUDIT, and DAST-10 screenings.
• Develop and monitor individualized care plans while addressing social needs with Community Health Partners and other care team members.
• Conduct clinical visits, follow-ups, medication reconciliation, administration, compliance support, and member education.
• Participate in case conferences, coordinate providers and specialized resources, and support chronic disease and co-occurring psychiatric disorder management.
• Close prioritized quality gaps and maintain thorough clinical documentation and ICD/CPT coding in electronic health records and care coordination platforms.

Requirements

• Graduate of an accredited school of nursing with an R.N. qualification.
• 3+ years of relevant work experience.
• Ability to conduct in-person clinical examinations and regular member-facing clinical visits.
• Ability to complete Cityblock’s medical clearance requirements, including immunity documentation for MMR, Hepatitis B, and Varicella and a TB screen, or obtain an approved medical or religious accommodation.
