Care Manager (Healthcare)

Location: Field-based in Colorado, covering Boulder, Larimer, Weld, Logan, Morgan, Denver, Jefferson, Broomfield, Gilpin, Clear Creek, Adams, Arapahoe, Douglas, Elbert, El Paso, Lincoln, and Pueblo counties

Compensation: $50 per completed field visit

What you will do

• Schedule and conduct quarterly supervisory visits required for home-care compliance.
• Perform home inspections to assess safety and compliance of participants’ homes and surroundings.
• Support members and caregivers with continuing care and appropriate use of authorized hours.
• Monitor caregiver daily notes for training or support needs and document contacts in Salesforce.
• Maintain virtual or in-person contact with caregivers according to service plans.
• Escalate concerns about health, safety, surroundings, or unmet needs to the staff RN or Case Manager.

Requirements

• Bachelor’s degree or equivalent work experience.
• At least two years of experience as a bedside CNA, QMAP, Direct Care Worker, Medical Assistant, or in a similar role.
• At least one year of call-center, telephonic outreach, customer-service, or comparable experience.
• Strong communication, rapport-building, problem-solving, critical-thinking, active-listening, and time-management skills.
• Ability to work independently, identify issues or trends, escalate concerns appropriately, and maintain patient confidentiality under HIPAA guidelines.
• Ability to conduct field visits throughout the listed Colorado coverage area.

Nice to have

• Field experience in a medical or case-management environment.
• Experience with data input, case review, case-note entry, and Salesforce.
