Program Manager | Enhanced Care Management

Location: Hybrid role based in the Inland Empire, California, with regular in-person presence at Chapparal Medical clinics, particularly in Pomona and Rancho Cucamonga, community outreach in the field, and occasional travel to Los Angeles. Candidates must reside locally in the region served.

Salary: $70,000–$85,000 USD per year

What you will do

• Supervise an interdisciplinary team of case managers, including registered nurses, care coordinators, and community health workers.
• Maintain a behavioral health caseload, conduct screenings and assessments, and develop individualized care plans with patients and the care team.
• Provide behavioral health guidance, warm handoffs, crisis response, and triage for suicidality, acute mental health concerns, and substance use disorder crises.
• Oversee caseload assignment, patient enrollment and disenrollment, care-plan implementation, documentation, and compliance metrics.
• Lead team operations through huddles, data reviews, care-planning meetings, quality-improvement initiatives, and quarterly objectives.
• Recruit, onboard, coach, mentor, and train case managers while building relationships with partner organizations and external stakeholders.

Requirements

• 4+ years of experience managing frontline staff.
• At least 2 years of experience in behavioral health case management, care coordination, or direct services with Medi-Cal or similarly complex populations.
• Deep understanding of Medi-Cal populations, including people experiencing homelessness, mental illness, or substance use disorders.
• Master’s degree in Social Work (MSW) or Marriage and Family Therapy (MFT).
• Strong organizational, communication, coaching, conflict-resolution, and data-driven decision-making skills.
• Valid driver’s license, automobile insurance, and reliable transportation.

Nice to have

• California licensure or license eligibility as an LCSW or LMFT.
• Experience with CalAIM, ECM programs, IEHP, or the Inland Empire county behavioral health system.
• Experience supporting people experiencing homelessness, justice involvement, or high emergency-department utilization.
• Bilingual English/Spanish communication skills.
