What you will do
- Coordinate with county and independent case managers on service authorizations, care plan updates, waiver transitions, reauthorizations, and appeals.
- Build relationships with Colorado Case Management Agencies through regular meetings, case reviews, and joint resolution efforts.
- Collaborate with case managers, patients’ families, clinical teams, and internal stakeholders to develop care plans aligned with clinical needs and Medicaid requirements.
- Provide expert guidance on complex cases involving multiple waivers, emergency authorizations, and appeals.
- Support authorization operations for Colorado HCBS waiver programs, including CFC, CES, and SLS, in accordance with HCPF policies and Colorado PAR requirements.
- Create training programs, mentor staff on LTHH waiver requirements, and establish performance metrics for approval rates and processing times.
Requirements
- 5+ years of experience coordinating with Colorado case managers in Medicaid LTHH settings.
- Documented success facilitating authorizations, reauthorizations, appeals, and continuity of care.
- Strong relationship-building, communication, problem-solving, and stakeholder management skills.
- Deep knowledge of Colorado’s LTHH ecosystem, HCPF policies, prior authorization operations, case coordination, and the ColoradoPAR system.
- Experience collaborating with patient families, clinical teams, and multiple case managers on continuous care coverage.
- Colorado Medicaid certification and LTHH authorization training are strongly preferred; a bachelor’s degree in Healthcare Administration, Business, or an equivalent field is preferred.
