RN Case Manager / Clinical Instructor

Salary: $80,000–$86,000 per year

What you will do

• Manage a diverse patient caseload and develop individualized care plans based on assessments and patient goals.
• Coordinate care with primary care providers, internal clinical teams, patients, and family caregivers.
• Coach and oversee Home Health Aides, educate patients and families, and ensure adherence to clinical standards.
• Maintain accurate electronic charting and complete required administrative documentation within 24 hours.
• Design and deliver skills training for Home Health Aide students, including in-person instruction at long-term care facilities.
• Monitor readmission rates and caregiver satisfaction, escalate clinical issues, and participate in approximately one weekend of on-call coverage per month plus holidays.

Requirements

• Associate's or bachelor's degree in Nursing.
• Current, valid RN license in Pennsylvania or an unencumbered compact/multistate license.
• At least 2 years of clinical RN experience, including at least 1 year in home health.
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to work flexible hours, including weekends, on-call rotations, and holidays.
• Strong communication and interpersonal skills.
