RN Care Manager - Baltimore - Hybrid - Mainly Remote

Location: MD - Remote, primarily telephonic with occasional travel to meet members in the field

Salary: $95,000–$102,000 per year

What you will do

• Manage a panel of clinically complex members across clinical programs, quality initiatives, and emergency and inpatient utilization pathways.
• Complete assessments and condition-specific and behavioral health screenings, including PHQ-9, GAD-7, AUDIT, and DAST-10.
• Develop and regularly reassess individualized care plans with members, addressing social needs with Community Health Partners.
• Conduct telephonic and in-person clinical visits, follow-ups, examinations, medication reconciliation, education, and compliance support.
• Coordinate care with the integrated Cityblock Care Team, external providers, community partners, and members during inpatient and emergency department transitions.
• Document care accurately in electronic health records and close quality gaps using ICD and CPT coding.

Requirements

• Graduate of an accredited school of nursing and licensed as a Registered Nurse.
• 3+ years of professional experience.
• Strong critical thinking, independent clinical judgment, and complex decision-making skills.
• Strong written and verbal communication across phone, text, virtual, and in-person settings.
• Experience or comfort engaging members remotely with technology.
• Ability to apply Motivational Interviewing and Trauma-Informed Care principles.
