What you will do

• Manage a caseload by completing assessments, developing care plans with patients and clinical teams, and carrying out planned activities.
• Use ScopeAI to support structured clinical assessments and surface insights for clinical decision-making with providers.
• Build rapport, coach patients on health risks, and promote behavioral change and self-management.
• Accompany members to medical and social service appointments and connect them with local, county, and state programs.
• Conduct outreach, enroll eligible patients in the Akido ECM Program, advocate for members, and follow up on care needs.
• Document every patient encounter in designated case management systems.

Requirements

• High school diploma or equivalent.
• 2+ years of experience working with CalAIM populations of focus, including people experiencing homelessness, adults with SMI/SUD, people transitioning from incarceration, or adults with complex medical needs.
• Ability to work independently and collaboratively in a dynamic outdoor environment while prioritizing competing tasks.
• Strong oral communication and interpersonal skills, including the ability to articulate relevant personal experiences.
• Ability to use computers and case management software for documentation.
• Travel of 50–75% is required; a valid driver’s license, automobile insurance, and reliable transportation are required.
