Community Health Field Worker

Location: Must reside in Central or Northern Louisiana. Combination of work at home and field work with local travel to meet members; occasional travel to hirify.global offices for training or meetings may be required. Monday–Friday, 8:00 AM–5:00 PM Central Standard Time; 40 hours per week.

Salary: $45,400–$61,300 per year.

What you will do

• Support Louisiana Medicaid enrollee care-management teams through in-person and telephone outreach.
• Assess healthcare needs, socioeconomic barriers, preferences, and home environments.
• Help schedule physical and behavioral health appointments and address barriers to attendance.
• Advocate for enrollees with providers, schools, community resources, and other partners.
• Monitor care-plan compliance, support medication and treatment adherence, and provide motivational interviewing.
• Deliver culturally appropriate health education, conduct community outreach, and identify gaps in local resources.

Requirements

• At least two years of experience working with community resources, community health agencies, or social service agencies.
• Intermediate working knowledge of Microsoft Office programs.
• Strong understanding of and respect for cultural and demographic diversity.
• Ability to advocate for members across all levels of care.
• Residence in Central or Northern Louisiana is required.
• Ability to work a 40-hour week on the stated Central Time schedule and travel locally to meet members.

Nice to have

• Bachelor’s degree in Social Work or a related field.
• Community Health Worker training or certification, or willingness to complete it within one year.
• Familiarity with state Medicaid program guidelines and experience engaging Medicaid enrollees with physical or behavioral health needs.
• Fluency in Spanish, French Creole, or Vietnamese.
• Residence in the listed Regions 6 or 8 Louisiana parishes.
