Location: Amador County, California, United States. Hybrid work: primarily in the community, with some work-from-home responsibilities. Travel to community events and in-person patient appointments is required, with reliable transportation. U.S. work authorization is required.
Salary: $27.00–$30.00 per hour. Seasonal temporary position ending in December 2026.

What you will do
- Register and enroll patients through community engagement, outbound calls, virtual outreach, and in-person intake.
- Support assigned patients with care navigation, appointment scheduling, prescription and lab support, referrals, care-plan adherence, and continuity of care.
- Assess social determinants of health, develop care plans, connect patients with community resources, and enroll them in support programs.
- Collaborate with multidisciplinary care teams to triage requests, coordinate clinical services, and resolve complex patient needs.
- Provide culturally and linguistically appropriate health education, motivational support, and social-resource referrals.
- Participate in community events, build relationships with community organizations, facilitate group education, and document interactions across multiple systems.
Requirements
- Fluent verbal and written English and Spanish.
- High school diploma or GED.
- Community Health Worker certification with field experience, or 2,000+ hours of CHW work in the past three years and willingness to obtain certification within 18 months.
- 1–3 years of healthcare or community healthcare-navigation experience and 2–5 years of community work, advocacy, engagement, or organizing.
- Ability to use multiple computer applications, learn new technologies, build effective working relationships, and train others.
- COVID-19 vaccination is required where legally permitted, unless an approved reasonable accommodation applies.
Nice to have
- Knowledge of Latino customs and cultural norms and strong ties to the local Latino community.
- Familiarity with local resources supporting social determinants of health.
- Google Workspace experience, EHR documentation experience, and motivational interviewing training.
