Outreach Specialist (Community Health)

Location: Bronx MIC Touchdown, New York; hybrid role with field visits throughout NYC and surrounding areas

Salary: $24.00–$28.00 per hour

What you will do

• Conduct virtual, telephonic, and field-based outreach to eligible members, spending most of the week meeting members in the field.
• Begin member intake, collect verbal and written consent, and facilitate handoffs to care teams.
• Review demographic information, document emergency contacts, track tasks, and enter visit notes in the Commons platform.
• Communicate urgent needs, member information, and safety concerns to care teams, members, and family members.
• Plan field visits, map travel routes, complete chart reviews, return calls, and submit weekly travel expense reports.
• Build Community Based Organization partnerships, support referrals, and achieve outreach, visit, activation, and quality targets.

Requirements

• Fluent English and Spanish.
• At least one year of experience in a similar role.
• Ability to travel throughout NYC and surrounding areas.
• Ability to conduct member outreach, document information accurately, coordinate intake, and identify safety concerns.
