RN Case Manager (AI Healthcare)

Location: Hybrid in Fort Collins, Colorado; must reside in or be willing to permanently relocate to the area

Salary: $80,000–$84,000 per year

What you will do

• Coordinate comprehensive care for a diverse patient caseload with primary care physicians, therapy providers, families, and internal support teams.
• Provide clinical oversight, coaching, and supervision to Certified Nursing Assistants and Home Health Aides.
• Develop, implement, and adjust individualized patient care plans.
• Maintain accurate electronic documentation with a target turnaround of under 24 hours.
• Educate and support patients and families on treatment plans and healthcare needs.
• Manage clinical escalations, participate in on-call and weekend rotations, and support phone triage coverage.

Requirements

• Associate's Degree in Nursing or a Bachelor of Science in Nursing.
• Current, valid, unencumbered Colorado RN license or valid Compact/Multi-state RN license required.
• At least 2 years of clinical experience as a Registered Nurse.
• Reliable transportation, a valid driver's license, and current auto insurance for local travel.
• Availability for flexible hours, local patient travel, on-call coverage, and weekend rotations.
• Must reside in or be willing to permanently relocate to the Fort Collins, Colorado area.

Nice to have

• At least 1 year of home health experience.
• Clinical case management or supervisory experience.
• Experience in pediatric or adult home care.
• Knowledge of Medicaid paid family caregiving programs and CNA scope of practice.
• Familiarity with OASIS documentation standards.
