Manager, Care Coaching- Jefferson (Healthcare)

Location: Remote from home, supporting Jefferson County, WI and surrounding areas/counties; residence within 2 hours of Jefferson, WI preferred. Occasional travel to hirify.global offices, in-person unit meetings, and quality visits may be required at least once per month.

Salary: $78,400–$107,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Lead, mentor, and support Field Nurses, Care Coaches, and IDT staff.
• Ensure delivery of Family Care benefits in compliance with the DHS contract.
• Identify learning needs, set expectations, and coordinate orientation and ongoing training.
• Assess staff competencies and performance standards, review documentation, and evaluate care-management processes.
• Assess utilization and quality of care management for members.
• Collaborate across departments, educate community partners, and participate in hiring, onboarding, training, meetings, and quality visits.

Requirements

• Bachelor’s degree in human services or a related field, or an associate degree in nursing.
• At least 1 year of leadership experience in the human services field.
• At least 3 years of experience working in the Family Care program.
• Ability to work Monday–Friday, 8:00 am–4:30 pm CST and travel at least once per month, with potential for additional travel.
• Ability to work from a dedicated home space that protects member PHI and HIPAA information.
• Home internet service with at least 25 Mbps download and 10 Mbps upload speed.

Nice to have

• Five years of experience working with individuals with intellectual or developmental disabilities, physical disabilities, or frail elders.
• Residence within 2 hours of Jefferson, WI.
