RN Nurse Care Manager - Middlesex- Hybrid

Location: Middlesex County, Massachusetts, United States; hybrid with approximately 70% field-based work and 30% remote work

Salary: $95,000–$105,000 per year

What you will do

• Manage a panel of clinically complex members and communicate program expectations, goals, and timelines.
• Complete clinical, self-efficacy, condition-specific, and behavioral health assessments, including PHQ-9, GAD-7, AUDIT, and DAST-10.
• Develop and regularly reassess individualized care plans, addressing clinical and social needs with interdisciplinary partners.
• Conduct in-person clinical examinations, regular visits, follow-ups, medication reconciliation, administration, compliance support, and education.
• Coordinate care with Cityblock teams, external providers, community resources, and inpatient and emergency department services.
• Address quality gaps and maintain accurate clinical documentation and ICD or CPT coding in electronic health records and care-facilitation platforms.

Requirements

• Graduate of an accredited school of nursing and licensed as a Registered Nurse.
• 3+ years of professional experience.
• Strong critical thinking, independent clinical judgment, problem-solving, and ability to identify barriers to care.
• Strong written and verbal communication across phone, text, virtual, and in-person settings.
• Experience or comfort using technology to engage members remotely and document care in electronic systems.
• Ability to apply Motivational Interviewing and Trauma-Informed Care principles.
