RN Care Manager

Location: Wilmington, North Carolina; up to 85% remote with occasional in-person field visits to members in New Hanover County or Brunswick County

Salary: $71,000–$90,500 annual

What you will do

• Manage a panel of clinically complex members and develop individualized care plans with them.
• Complete clinical, self-efficacy, condition-specific, and behavioral health assessments, including PHQ-9, GAD-7, AUDIT, and DAST-10.
• Coordinate chronic disease management, behavioral health support, health maintenance, and transitions from inpatient and emergency care.
• Conduct clinical visits and follow-ups, including appropriate in-person examinations and medication reconciliation, administration, compliance, and education.
• Collaborate with the integrated care team, external providers, community partners, and case conferences.
• Close quality gaps and maintain accurate clinical documentation and ICD or CPT coding in electronic health records and care-facilitation tools.

Requirements

• Graduate of an accredited school of nursing and registered as an R.N.
• 3+ years of professional experience.
• Strong critical thinking, independent clinical judgment, and problem-solving skills.
• Strong written and verbal communication skills across phone, text, virtual, and in-person settings.
• Experience or comfort using technology to engage members remotely, document interactions, and coordinate care.
• Ability to apply Motivational Interviewing and Trauma-Informed Care principles.
