Associate Director, SNP Care Management

Location: Must reside in Michigan; hybrid home/office work with up to 25% travel within Michigan. Schedule is Monday through Friday, 8:00 AM–5:00 PM Eastern Time, with occasional overtime and travel to hirify.global offices for training or meetings.

Salary: $104,000–$143,000 per year, plus eligibility for a bonus incentive plan.

What you will do

• Lead, coach, and develop care management leaders responsible for frontline associates.
• Drive performance, quality, productivity, engagement, inclusion, and member experience across multiple teams.
• Analyze operational, staffing, productivity, and outcome data to guide decisions and improvements.
• Partner with Workforce Management and operational stakeholders on staffing, scheduling, inventory, and capacity planning.
• Implement initiatives that generate administrative savings while maintaining quality and compliance.
• Lead cross-functional projects and resolve complex operational and performance challenges.

Requirements

• Active Registered Nurse license in the state of Michigan.
• Must reside in Michigan.
• At least 6 years of experience in clinical care, care management, or population health.
• At least 2 years of leadership experience managing leaders or care management professionals in a clinical, managed care, or population health environment.
• Experience leading multiple projects from development through implementation and presenting operational and performance data to stakeholders.
• Knowledge of Home and Community-Based Services, Long-Term Services and Supports, Medicaid regulations, and person-centered planning.

Nice to have

• Advanced degree in nursing or a business-related field.
• Progressive operational leadership experience in healthcare, care management, population health, or managed care.
• Experience leading managers or supervisors in a multi-level leadership structure.
• Experience with workforce planning, productivity management, and operational performance improvement.
