Enrollment RN (PACE Healthcare)

Location: San Leandro, California, with travel to participants’ homes, care environments, work sites, and community locations throughout the service area.

Salary: $50.00–$61.00 per hour, plus a target annual bonus and possible additional incentive compensation.

What you will do

• Serve as the clinical lead throughout the participant enrollment process in partnership with outreach, enrollment, and interdisciplinary teams.
• Conduct nursing, clinical eligibility, Nursing Facility Level of Care (NFLOC), and home safety assessments in participants’ homes and other care environments.
• Present clinical findings in enrollment eligibility meetings and support informed enrollment recommendations.
• Educate prospective participants and caregivers, answer clinical questions, and build trusted relationships.
• Maintain accurate records in electronic health records and enrollment documentation systems.
• Support outreach, community engagement, workflow improvement, and operational effectiveness.

Requirements

• Associate’s degree in Nursing from an accredited program or equivalent nursing education.
• Active, unrestricted Registered Nurse license in the applicable state.
• At least one year of nursing experience with older adults, frail elderly populations, or people with complex medical needs.
• Experience conducting comprehensive clinical and home safety assessments, applying sound clinical judgment, and managing multiple priorities.
• Valid driver’s license, reliable personal transportation, required automobile insurance, and ability to travel throughout the service area.
• Active CPR/BLS certification or ability to obtain it within 30 days; required TB screening, immunizations, infection-control compliance, and participant-facing health requirements apply. Employment authorization in the United States is verified through E-Verify.

Nice to have

• Bachelor’s degree in Nursing.
• Experience in PACE, Medicare, Medicaid, managed care, or other value-based healthcare environments.
• Knowledge of PACE, CMS, DHCS, and federal or state requirements related to eligibility, enrollment, and care coordination.
• Experience with NFLOC, home safety, or other clinical eligibility assessments.
• Bilingual Spanish, Mandarin, or Cantonese.
