Wellness Coordinator (Behavioral Health)

Salary: $32–$37 per hour plus bonus structure.

What you will do

• Lead and manage the Health Home team supporting opioid treatment program patients.
• Conduct outreach, assess patient needs, and develop and update individualized care plans.
• Coordinate care management, preventive services, mental health and substance use services, discharge planning, and transitions of care.
• Connect patients with community resources, social support, long-term care, and recovery services.
• Facilitate Medicaid and Health Home enrollment, educate new patients, and provide advocacy and problem-solving support.
• Monitor service documentation and billing, maintain provider agreements, and oversee quality improvement activities.

Requirements

• Master’s degree in Social Services, Psychology, Social Work, or Criminology, or an active Registered Nurse license.
• Familiarity with behavioral health and related professional experience.
• Strong integrity and communication skills for engaging with people from diverse backgrounds.
• Structured approach to managing high caseloads.
• Availability for an early schedule: Monday through Friday, 5:00 a.m. to 1:00 p.m.
• Ability to work on-site in Johnston and at other hirify.global Rhode Island locations as needed.
