Field Care Manager (Behavioral Health)

Location: Must reside in the Eastern Kentucky area; field visits in homes, community settings, hospitals, shelters, and correctional facilities, with quarterly meetings in Louisville, KY. Travel required up to 75%, approximately 30–40 miles per day.

Salary: $65,000–$88,600 per year, plus eligibility for a bonus incentive plan.

What you will do

• Conduct comprehensive behavioral health assessments and evaluate members’ behavioral, physical, environmental, and psychosocial needs.
• Develop member-centered care plans with measurable outcomes and adjust interventions based on progress.
• Coordinate services across healthcare providers, behavioral health professionals, and community resources.
• Support reentry and care transitions, including discharge from incarceration.
• Conduct field visits and telephonic outreach to identify barriers and improve health outcomes.

Requirements

• Residence in the Eastern Kentucky area is required.
• Master’s degree in behavioral health or a related field.
• Active, unrestricted Kentucky licensure as an LCSW, LPCC, LMFT, another independent Kentucky behavioral health license with at least one year of post-degree clinical experience, or an RN license with a bachelor’s degree in nursing and at least three years of behavioral health experience.
• At least two years of case management or care management experience.
• Experience working with adult populations.
• Valid state driver’s license and personal vehicle liability insurance are required; the role is patient-facing and requires TB screening.

Nice to have

• Experience with Kentucky Medicaid, managed care, high-risk populations, SPMI, or SUD.
• Certified Case Manager certification.
• Knowledge of motivational interviewing, trauma-informed care, behavior change, health promotion, wellness coaching, and member engagement.
• Bilingual English and Spanish skills, including the ability to speak, read, write, interpret, and explain documents in Spanish.
