Nurse Practitioner (Central New Jersey)

Location: Field - New Jersey; must be located in or near Mercer, Middlesex, or Somerset County, New Jersey, and able to travel locally for home visits.

Salary: $140,000–$165,000 USD per year

What you will do

• Deliver ongoing, longitudinal primary and supportive care in members’ homes.
• Manage a dedicated patient panel with complex and chronic medical conditions.
• Develop and maintain care plans focused on continuity, quality, and outcomes.
• Work with a high degree of clinical autonomy while building lasting patient relationships.
• Participate in a shared on-call rotation to provide continuous member support.

Requirements

• Must be located in or near Mercer, Middlesex, or Somerset County, New Jersey, with local travel for home visits.
• Active and unencumbered New Jersey Registered Nurse license required.
• Certified Advanced Practice Nurse Practitioner (APN).
• Master’s degree or post-master’s certificate in advanced clinical practice.
• At least 1 year of Nurse Practitioner experience in a clinical setting.
• Active DEA and CDS registration and current Basic Life Support (BLS) certification.
