Nurse Practitioner (Remote, NJ License Required)

Location: Remote within the United States; active New Jersey license required.

Salary: $75/hour regular rate; $85/hour premium rate.

What you will do

• Provide phone- and video-based follow-up care for patients recently hospitalized and discharged.
• Support the Transitions of Care program through telephonic outreach and scheduled visits.
• Complete medication reconciliation, symptom screening, and orders for follow-up labs, medications, and supplies.
• Build patient trust, explain the care model, and connect patients with appropriate services.
• Coordinate care across hospitals, emergency departments, pharmacies, and community providers.
• Collaborate with nursing and administrative colleagues while contributing to individual and population health goals.

Requirements

• Certification as a Nurse Practitioner in Adult, Family, or Geriatric practice.
• Active New Jersey state licensure required, with the ability to obtain additional state licenses.
• 5+ years of post-graduate clinical experience in primary care or geriatrics; up to 2 years may be substituted with skilled-nursing or long-term-care experience.
• Direct experience managing complex, comorbid Medicare and Medicaid populations.
• Motivational interviewing, strong written and verbal communication, collaboration, and problem-solving skills.
• Comfort with virtual care delivery and digital-first workflows.

Nice to have

• Active licenses in TX, PA, IL, NY, MA, MN, LA, or SC.
• Bilingual English and Spanish communication skills with cultural fluency.
• Familiarity with Google Suite and Slack.
