Nurse Practitioner (AI Healthcare)

Location: San Francisco Bay Area, California; field-based work with travel up to 75% of work time

What you will do

• Provide frontline acute, chronic, and preventive care to patients experiencing homelessness, behavioral health challenges, serious mental illness, substance use disorders, complex chronic conditions, and transitions from incarceration.
• Conduct patient assessments, collect medical histories, perform physical examinations, order and interpret diagnostics, and prescribe treatment within the scope of practice and state regulations.
• Develop and implement care plans that support patient stability and improved health outcomes.
• Exercise independent clinical judgment and collaborate with physicians and multidisciplinary care teams on complex cases.
• Coordinate care with physicians, nurses, social workers, outreach staff, and clinical support teams.
• Document patient encounters accurately to support continuity of care.

Requirements

• At least 2 years of experience working with CalAIM populations of focus and prior clinical experience; new graduates are not eligible.
• Experience in street medicine or related settings, such as homeless shelters or outreach programs, and familiarity with Bay Area resources for underserved populations.
• Active, unrestricted California RN license and active California Nurse Practitioner certification.
• National Nurse Practitioner board certification, California Furnishing Number, DEA registration or eligibility, NPI number, and BLS certification.
• Valid California driver’s license and a reliable, insured vehicle required for field travel.
• Ability to work independently within clinical regulations, coordinate support staff, and provide care in dynamic outdoor settings.
